DC Adoption Scholarship (DCAS) (Cont.)
 
The following documents, which must be mailed or hand delivered, at the same time, are required to complete the application process for DC Adoption submission to Office of the State Superintendent of Education (OSSE).  See "Know the Required Supporting Documents" below for more information.
 
Required Supporting Documents
For Both Adopted and 9/11 Applicants
  • Original, signed DC OneApp Supporting Documents Checklist and Agreement
  • One Student Aid Report (SAR), PDF or eight (8) page paper version, as provided by the US  Department of Education after the completion of the Free Application for Federal Student Aid (FAFSA) 
  • New applicants must provide proof of high school completion (i.e. high school diploma, General Equivalency Diploma (GED) certificate, college acceptance letter or college transcript) (Current high school students may provide a signed Satisfactory Academic Progress Toward Graduation (SAP) Form with original signatures.  The SAP Form can be found in the DC OneApp 'Part 3: Sign & Mail Forms & Documents.")
  • Affirmation and Legal Disclaimer Statement with original signature from the applicant and the parent/legal guardian (if applicant is a dependent) (The Affirmation and Legal Disclaimer Statement can be found in the DC OneApp "Part 3: Sign & Mail Forms & Documents.") 
For Adopted Applicants
 
  • Copy of the final adoption decree
  •  
    Note: All required supporting documents must be mailed or hand delivered to HEFS at the same time to be considered for an award.
